One of the most important steps while doing a project is to identify the stakeholders and keep continuous communication and collaboration. The project team and the stakeholders work as a team and treat each other concerning attaining a common goal to see the positive outcome and therefore raising the standard of care. Stakeholders have different experiences, skills, and ideas which will make the team strong to intervene in the new change process.
My DNP project will be by doing the case study and implementing the evidence-based intervention. This practice problem was brought to my attention when I was working as a nursing supervisor in a long-term care facility for the elderly population. Most of the patients were diagnosed with Type 2 diabetes and often they get transferred to the hospital due to complications of diabetes. The readmission rates were seen much higher and there is a high range of hba1c noted on these patients. As a DNP scholar, my idea is to implement a diabetes self-management program for these patients which will focus on diet, exercise, and medication adherence. Doing these interventions will reduce the readmission rate and decrease hba1c levels in type 2 diabetes patients. The organizational culture brings an awareness of how the team and the patients function towards a common goal and the safety of everyone in the organization (Gorzelany et al., 2021). Some organizational barriers that may be present in this project are the patients not getting enough knowledge due to lack of resources, secondly, the nurses not being enthusiastic to implement this new change in practice. Change in practice is tough but can be implemented if the appropriate rationale and evidence-based literature reviews are provided to the team (Ling et al., 2019).
For my DNP project, the implementation plan includes four milestones.
Milestone 1 is the planning phase where the roles are assigned, do a team collaboration and communication, collecting data, finding out resources, and setting up the project timeline. Working with the project navigator, identification of the practicum site, sharing the practice problem idea, IRB Pre-screening review form, and acceptance on the PICOT question and the project.
Milestone 2 is the communication phase, at this time there should be clear communication between the DNP scholar and the projector, and the practicum leaders.
Milestone 3 is called the implementation and management phase, in this phase conducting a thorough literature review, identifying the leaders within the team to educate and prepare to implement the intervention, collecting data on pre-intervention hba1c scores, ongoing communication, and exchanging the data’s to the team and the administrative team.
Milestone 4 is the evaluation phase where the collection of post-intervention hba1c level scores are evaluated, analyze the outcome, and discuss with the project partners to implement the change in the current practice.
